Streszczenie
Protezy typu overdenture (OVD) oparte na implantach są alternatywą w leczeniu bezzębnych pacjentów, szczególnie w przypadkach, w których retencja i stabilizacja protez całkowitych konwencjonalnych jest niewystarczająca, występuje słaba jakość i niewystarczająca ilość kości lub istnieją ograniczenia finansowe, aby wprowadzić więcej niż 4 wszczepy.
Elementami utrzymującymi mogą być połączenia belkowe, zaczepy kulkowe, lokatory, magnesy oraz systemy koron teleskopowych.
W pracy przedstawiono wady i zalety różnych rozwiązań, uwarunkowania kliniczne, czynniki ryzyka oraz aspekty projektowania. Zaprezentowano etapy postępowania chirurgiczno-protetycznego przy wykonywaniu protez overdenture w przypadku bezzębia szczęki i żuchwy.
Zwrócono uwagę na diagnostykę przedzabiegową niezbędną do uzyskania zadowalającego efektu funkcjonalnego i estetycznego. Wykonane protezy przywróciły utraconą sprawność układu stomatognatycznego, a także poprawiły funkcję żucia, mowy, estetyki i komfortu psychicznego pacjentów.
Abstract
Implant-retained overdentures (OVD) are a treatment option for patients who are not satisfied with conventional complete dentures, especially in cases where conventional complete dentures provide insufficient retention and stability, the bone is of poor quality or inadequate volume, or the patient is unable to cover the financial cost of having more than 4 implants placed. The present study discusses the advantages and disadvantages of particular solutions, the clinical conditions, the risk factors and design issues. The study describes the different stages of the prosthetic-surgical procedure for making an implant-supported overdenture for patients with edentulous mandibular and maxilla. Attention is paid to the pre-operative diagnostics, which is essential to ensure satisfactory functional and aesthetic effects. The dentures restored the lost efficiency of the stomatognathic system and also improved the masticatory function, speech, aesthetics and mental comfort of the patient.
