Patologiczna migracja zębów (pathologic tooth migration, PTM,) to współistniejące z chorobą przyzębia przemieszczenie zębów, wynikające z zaburzenia równowagi między czynnikami fizjologicznymi, takimi jak stan tkanek przyzębia, siły wyzwalane przez język, wargi, policzki oraz kontakty okluzyjne. W wielu przypadkach PTM jest powikłaniem stanów zapalnych toczących się w przyzębiu. Patologicznej migracji często ulegają zęby sieczne, czego skutkami są ich wachlarzowe wychylenie, wydłużenie koron klinicznych oraz diastema. W zależności od stadium PTM podejmuje się różne decyzje lecznicze.
W pracy omówiono etiologię, leczenie i rodzaje patologicznych migracji zębów, ze szczególnym uwzględnieniem migracji zębów siecznych. Przedstawiono również trzy przypadki kliniczne pacjentów z PTM w obrębie zębów przednich wraz z wdrożonym paliatywnym postępowaniem terapeutycznym, wynikającym z niezgody pacjentów na zalecone leczenie periodontologiczno-ortodontyczne.
Pathologic tooth migration associated with periodontal disease – diagnosis and treatment in material of the authors
Pathological tooth migration (PTM) co-exists with the migration of teeth and periodontal disease, resulting from an imbalance between physiological factors, such as the condition of the periodontal tissues, the forces released by the tongue, lips, cheeks and occlusal contacts. In many cases, PTM is a complication of periodontal inflammation. Incisor teeth are frequently subjected to pathological migration, whose consequences are fan-like inclination, elongation of clinical crown and diastema. Depending on the stadium of the PTM various treatment decisions are undertaken.
The study discusses the aetiology, treatment and types of pathological tooth migration, with special emphasis on migration of incisors. Also, three clinical cases of patients with PTM in the region of the anterior teeth are described, together with the implemented palliative therapeutic procedures, because patients did not agree to the periodontal-orthodontic treatment that was advised.
