Evaluation of surgical treatment outcomes of coronally advanced flap and tunnel technique in management of class I and II multiple gingival recessions in single rooted maxillary teeth

Streszczenie
Cel. Celem badania było porównanie efektów klinicznych chirurgicznego leczenia recesji dziąseł klasy I i II przy zębach jednokorzeniowych szczęki z wykorzystaniem techniki płata przesuniętego dokoronowo bez cięć pionowych (CAF) lub techniki tunelowej (TUN) w połączeniu z przeszczepem tkanki łącznej (sCTG).
Materiały i metody. W badaniu wzięło udział 27 pacjentów z mnogimi recesjami I/II klasy w szczęce, u których wykonano pokrycie recesji techniką CAF lub TUN. U pacjentów oceniano wybrane parametry kliniczne przed leczeniem i po 6 miesiącach od leczenia. Estetykę wyników leczenia oceniano, stosując skalę RES (Root Coverage Esthetics Score).
Wyniki. Do grupy CAF włączono 13 pacjentów (78 recesji), a do grupy TUN 14 pacjentów (79 recesji). Całkowite pokrycie recesji w CAF uzyskano w 84,6% przypadków, a w TUN w 91,1% przypadków. Średnie pokrycie recesji wynosiło odpowiednio 92,2% i 97,78% (p < 0,05). Punktacja w skali RES wynosiła 8,59 dla CAF i 9,61 dla TUN (p < 0,01).
Wnioski. Obie techniki w połączeniu z sCTG okazały się skuteczne w leczeniu mnogich recesji dziąsłowych zębów szczęki, ale bardziej estetyczne wyniki uzyskano w grupie TUN.

Abstract
Aim.
The aim of this study was to evaluate outcomes of surgical management of class I/II gingival recessions in single rooted maxillary teeth treated with coronally advanced flap (CAF) and tunnel technique (TUN) combined with subepithelial connective tissue graft (sCTG).
Materials and methods. A total of 27 patients with multiple class I/II gingival recessions were treated with CAF or TUN. Clinical evaluations was performed at baseline and 6 months after treatment. Esthetic outcomes were assessed with Root Coverage Esthetic Score (RES).
Results. 13 patients were included in CAF group (78 recessions) and 14 were included in TUN group (79 recessions). Complete root coverage was achieved in 84,6% for CAF and in 91,1% for TUN. Mean root coverage was 92,2% and 97,78%, respectively (p < 0,05). RES scoring was 8,59 for CAF and 9,61 for TUN (p < 0,01).
Conclusions. Both techniques combined with sCTG were effective in treatment of multiple gingival recessions in maxillary teeth. However, outcomes were more esthetic in TUN group.
